This award celebrates Primary Care teams that are leading the way in implementing best practices for people living with Type 2 diabetes in their everyday clinical care.

Who Can Apply?

• Any healthcare professional can apply on behalf of their own Primary Care team.
• You may also nominate another Primary Care team—with their consent.

What We’re Looking For

We invite submissions that highlight:

• Innovative approaches to Type 2 diabetes management in Primary Care
• Tangible improvements in patient outcomes
• Sustainable practices that can inspire and be replicated by others

How to Apply

To enter, complete the submission form by answering a few key questions about your initiative, team, and outcomes. Supporting materials (e.g., publications, reports, links) may be included but are optional.

Timeline

• 1 October, 2025: Award submissions open
• 15 November, 2025: Award submissions close
• January 2026: Awardee informed
• 11-14 March, 2026: Award delivery during the ATTD 2026 Award Ceremony
